This Year's Tournament

• Will be at the Moonlake Golf and Country Club, Tuesday June 15th
• 27 Holes, Texas Scramble Shotgun Start at 12:15 pm
• Driving Range opens at 10:00 am
• Registration at 10:30 am
• Lunch at 11:30 am
• Direct West Banquet of Wishes to follow

Includes: lunch, gift, door prize entry, round of golf with cart, banquet ticket, and a $100.00 tax receipt

Mail, Fax, or drop off your Registration Forms and Payment at the Children’s Wish Foundation Office (contact information at the bottom of this page). Payment is due with Registration.
